# The assessment is a Typeform embed — carry it over, do not rebuild it

Source: live HTML of `/our-approach/legal-health-check/` inspected 2026-09-02. The proposal
(2026-07-30) had listed "what platform powers the assessment form" as a week-one unknown
because the page's CTAs point at an anchor rather than a visible embed. This resolves it.

## What is on the live page

- The Business Legal Health Assessment (name to be confirmed) is a **Typeform** widget:
  `data-tf-widget="QIIINVqG"`, `data-tf-medium="snippet"`, `data-tf-opacity="100"`,
  `data-tf-iframe-props="title=BHA Survey"`, `data-tf-transitive-search-params=""`.
- Hidden fields: `data-tf-hidden="utm_source=,utm_medium=,utm_campaign=,utm_term=,utm_content="`
  — the five standard UTM parameters are passed into the Typeform as hidden fields (empty by
  default; Typeform fills them from the page URL). This is the existing source-attribution
  mechanism; losing it would break AISV's lead-source reporting.
- The embed sits inside `id="bha-form"`; the page's own CTAs link to `#bha-form`. The embed
  markup appears twice in the page source (a mobile/desktop or repeated-section duplicate).
- The page frames the assessment as three pillars: Spirit / Mind / Body.
- The page's disclaimer names Washington, Idaho, and Oregon as licensed states (see
  `.memory/naming-conflicts.md` — do not carry that sentence over unverified).
- Related legacy pages that may be Typeform result destinations:
  `/business-health-assessment-download/`, `/recommendations-summary-form/`,
  `/recommendations-summary-download/`. Whether Typeform delivers results to them is to be
  confirmed by Rian with AISV.

## Why it matters

The proposal sells placement, prominence, and instrumentation of the EXISTING assessment as the
site's "front door" (AISV's Phase 01 "BHA as the front door") — a dedicated page and a clean path
in, rather than an anchor link partway down a page — plus the assessment as the transitional CTA
paired with "Book a Consultation" everywhere. It explicitly does NOT include "rebuilding the
Business Health Assessment as an interactive AI-driven tool": that is AISV's **Creative Concept
03, "The Smart BHA"** (an AI-driven assessment that adapts to each visitor, routes qualified
responses into Pipedrive by persona, and packages each profile for the pitch), a project in its
own right. The platform reply to the client also noted a Next.js app would fit that future piece
and it can be added later without redoing the site.

## How to apply

1. Re-embed the same Typeform id `QIIINVqG` on the new assessment page with the same hidden
   UTM fields. Do not create a new form, a Gravity Forms clone, or a Kadence form for it.
2. Give it its own page under the confirmed name (nav entry, header CTA, end-of-post CTA,
   homepage assessment section from the brand story). The `#bha-form` anchor pattern is what we
   are replacing.
3. Fire a GTM event for the transitional path: on the Typeform submit event if the embed exposes
   one, otherwise on the CTA click. Track it separately from the "Book a Consultation" path.
4. To be confirmed before launch (Rian with AISV/the client): who owns the Typeform account, what
   the form routes to (Pipedrive, notifications, the download pages above), and the final name.
   Never ask for the Typeform login in chat.
5. If anyone proposes rebuilding the assessment logic in this build, point them at this file:
   it is out of scope, by the proposal's "Not included" list.
